Clostridium difficile is a component of the human colonic flora. If the physiological bacterial flora in the colon is damaged by administration of antibiotics, especially of clindamycin, fluoroquinolones, cephalosporins, or amoxicillin/clavulanic acid, C. difficile is able to multiply and to cause antibiotic-associated diarrhoea and pseudomembranous colitis. Severe cases might progress to toxic megacolon and end fatally. C. difficile can cause antibiotic-associated diarrhoea and a possibility of outbreaks in hospital settings warrants molecular typing.
This assay was designed for high-throughput typing. It also includes the detection of toxin genes (tcdA/B, cdtA/B), genes related to antimicrobial resistance, the slpA gene and additional variable genes.
For this assay, DNA preparations from colony material are subjected to multiplexed linear amplification and labelling with dUTP-linked biotin. Amplicons were hybridized to microarrays providing information on the presence of target genes and on their alleles.
The current software and database allows assignment of isolates to 40 different hybridisation profiles. Profiles cluster into 6 distinct clades that also can be corroborated by multilocus sequence typing.
The microarray based assay facilitates rapid and high-throughput genotyping of clinical C. difficile isolates including toxin gene detection and strain assignment.
